Thanks for this. My classes love things like this. I have something similar for a few tenses. I have left lots of blanks and have made the pupils search for the correct conjugations of various verbs and have some exercises included. That way I know for sure they have used it! :)

I'm a French native and you can either say "vous faites" or "vous faisez" so what you have in your worksheet is not wrong (unlike what previous commentors said).
Anyway, this resource is superb and will be very useful for my classes!
I too am a native French and I am not so sure about faisez. As far as I can remember, I have never seen faisez. When I was in school (25 years ago) we learned faites and were told faisez was wrong. The Bescherelle, Bled and (probably most?) dictionaries say "faites" and not "faisez". Mon Bescherelle (qui date un peu il est vrai: 1990!) va jusqu'à qualifier 'vous faisez' de barbarisme. Peut-être que ça a changé depuis? En tout cas, tous les conjugueurs ne retiennent que faites.

If I may though, there's a mistake with vous+faire, it should be faites
